Click the Enter New Record  button to initiate your electronic medical record.

  The corporate client must enter the CIC (corporate identification code) and Plan name to allow initial access to the electronic record.

  The record is currently blank. Fill out each section to the best of your knowledge. Please do not hurry the session. Once you submit the record, the record can be accessed by your Log On ID and your passcode. The Log On ID must be at least eight characters long and can be any letter or number combination as a user ID. The passcode is any four digit set. This can be changed as necessary for privacy.

  The first session requires that all bold items be completed before the record can be saved and initialized. Failure to do this will prevent the record from being saved! Starred items are very important for completeness. The sytem will remind you of missing items on future Log On activity.Updates can be recorded through the Client Login button.
